LPN Medical/Surgical

The LPN collaborates to deliver direct and indirect patient care in inpatient and/or ambulatory dialysis units under supervision of a physician or RN. Accurately assesses patients, applies clinical interventions, evaluates outcomes, and updates the plan of care in collaboration with the provider. Administers and documents medications per policy and best practice, and performs nursing procedures within scope. Collects data for assessments, participates in care planning and development of nursing interventions, and monitors responses to care. Provides patient education materials and reinforces health teaching, while directing questions to the RN/provider. Supports departmental development and onboarding, ensures accurate documentation, and participates in ongoing training and meetings.
