Field Care Manager, LTSS (RN) - Local Travel Required

Provide field-based care management for LTSS members, conducting in-home assessments and developing care plans within a regulated, managed care framework. Manage waiver enrollment and disenrollment, coordinating with members, caregivers, physicians, and the interdisciplinary care team. Monitor care plan effectiveness, document interventions, and adjust goals to ensure progress toward desired outcomes. Integrate behavioral health services and LTSS with home-and-community resources to ensure seamless, ongoing care. Conduct medication reconciliation as needed, support care coordination, and communicate clearly with members and care networks. Must travel locally (approximately 25-40%), demonstrate strong MS Office skills, and work independently in a dynamic environment.
