Weekend Home Care Registered Nurse

Provide one-to-one professional nursing care to hospice patients in the home, prescribed by a physician and compliant with state nurse practice acts. Lead patient assessment, care planning, therapeutic interventions, and overall supervision of care outcomes as the hospice case manager. Anticipate and manage symptoms in collaboration with the physician and interdisciplinary team; address caregiver grief and support the plan of care. Support Hospice Aides and LPN/LVNs; participate in weekly interdisciplinary team meetings and coordinate with the primary care physician or medical director as needed. Weekend scheduling options include Fri–Sat/Sun 12-hour shifts or Sat–Sun–Mon 12-hour shifts or 4-10s; sign-on bonus up to $10,000 for eligible external candidates. Minimum requirements: RN licensure in the state; at least 1 year in professional acute care; home care and EPIC experience preferred; strong communication and flexible, patient-centered mindset.
