Hospice RN Case Manager - Home Care

Provide one-to-one hospice nursing care in patients' homes as the RN Case Manager. Lead a patient caseload and coordinate with an interdisciplinary team in weekly meetings. Collaborate with primary care physicians and hospice medical directors to manage symptoms and care plans. Perform comprehensive assessments, care planning, and symptom management with a focus on end-of-life care.
