Medical Social Worker, Hospice - San Francisco

Conduct initial and ongoing psychosocial, emotional, spiritual, and bereavement assessments for hospice patients and their families. Provide short-term crisis intervention and individual or family counseling as needed. Collaborate with the Interdisciplinary Group (IDG) to develop and implement goal-directed care plans. Maintain timely documentation and coordinate referrals to community resources and services. Develop partnerships, deliver education/outreach, and support interagency cooperation to optimize patient care. Participate in staff development, case conferences, and ensure care standards compliance in the San Francisco area.
