Staff RN - Hospice

Provide compassionate, patient-centered hospice nursing care in patients' homes or care facilities, guided by an individualized Plan of Care. Serve as the primary contact for patients and families, guiding them through the hospice journey. Perform initial and ongoing assessments, monitor condition, and adjust care as needed. Collaborate with an interdisciplinary team to develop and implement personalized care plans. Educate and support caregivers, offering practical training and bereavement guidance. Maintain accurate documentation, ensure infection control and compliance, and support quality improvement initiatives.
