Job Coach

The Job Coach delivers intensive on-the-job training and ongoing coaching to rehab consumers transitioning into competitive or supported employment. Provides transportation guidance and workplace readiness training to facilitate successful job commutes and retention. Maintains weekly case documentation and monthly LTFA notes, tracks service hours, and submits activity reports for additional hours. Performs case management by linking trainees to community resources and other service providers to enhance community integration. Trains and supports co-workers and supervisors on effective collaboration with the placed consumer and participates in job development activities. Conducts pre-placement activities, coordinates referrals, and participates in audits, meetings, and ongoing departmental duties.
